If you are joining us from another clinic, there is also a records release form you can complete so we can request your previous dental information on your behalf. This helps us get a clearer picture of your history and ensures nothing important is missed.
You are welcome to complete everything at home at your own pace, or if you prefer, you can fill them out when you arrive a little early. Either way, we will guide you through it and make sure it feels straightforward.
